Whether for yourself or a loved one, we will take care of it.
We offer two core services
As your loved ones age, changes may come gradually. As these changes occur, it is understandable that you become concerned about their physical health, the condition of their residence or their overall vulnerability. Other times, a health crisis can turn their living situation upside down creating a feeling of uncertainty. A CareLink Care Manager can help you make sense of diagnoses, payment options, living accommodations and the dozens of other decisions that are coming your way.
CareLink is a program affiliated with the Ruth & Norman Rales Jewish Family Services (JFS), a premier social services agency who has been serving south Palm Beach County for over 40 years. Our Care Managers can help you get connected to our programs such as care giving support, respite, senior companion, non-medical home care, mental health counseling, dementia expertise, a vibrant senior center – and more.
What if life feels more complicated after you retire? Coordinating medical appointments, supervising in-home services, following up on paperwork and considering housing options all can seem overwhelming. What if you had a care concierge to help you navigate all of your choices?
One of our experienced CareLink Care Managers will meet with you to determine your needs and to develop solutions that address your priorities. We’re here to support you, enabling you to be as safe, healthy and independent as possible.
Need more help?
Here are some common questions that may help.
I’ve never heard of a care manager before. Why would my family hire a care manager?
Until you start to navigate healthcare or long-term care with an aging relative, most of us are unaware about the complexity of systems and benefit programs involved with aging. Many people understand that they need to hire tax preparers, realtors or other specialists to help with projects that require specialized knowledge. Likewise, a geriatric care manager has the knowledge and experience to help you sort through your options regarding health care and housing.
My parents are not very open to accepting help. How do you start the conversation about services?
We know that it can be difficult for anyone to admit that they need assistance. Older adults may fear a loss of independence or control when difficult decisions come up. Our goal is to assure each individual that we work towards their goals of safety, independence and optimal health. Respect is the cornerstone for our communications.
